Which Provider Should I Make My Appointment With?
We offer appointments with our Medical Provider as well as our Chiropractor. When you call to make your appointment our well trained staff will help you decide which provider you should initially see.
How Should I Prepare for My First Appointment?
Please download and complete new patient forms prior to your first appointment. You can bring them into out office with you or you can fax them to our office at 678-714-3053.
For Insurance patients, please be sure to bring your insurance card and drivers license with you to your appointment so we can verify your benefits.
If you have recently seen another medical provider for this injury or condition, please also bring with you your MRI or NCV’s reports, x-rays and any medical records that can help our doctors create the most beneficial treatment program for you.
How Long Should I Expect My Appointment to Last?
Your initial appointment should last 30-60 minutes depending on the complexity of the injury. Follow up appointments for physical therapy services should last between 30-45 minutes. Follow up chiropractic appointments should last between 15-30 minutes.
Do You Accept Insurance?
We accept most insurance plans, however as each insurance company offers multiple types of plans, so we will verify your individual benefits at the time of service. Then, following your consultation with our providers we will discuss your insurance benefits and provide you with a care plan.
